CONVENTION REGISTRATION

Please note that all participants over the age of 4 years must register and pay the registration fee. Participants and spouses or guests who wish to earn CME credit must pay the full convention registration fee for a NAAMA member or non-member, as applicable.

As an alternative to online registration you may also download a copy of the PDF convention booklet and fill out pages 6 and 7 by hand. Your completed application must be mailed to NAAMA or faxed to (248) 646-0617.

*The registration fee entitles members, non-members, medical students and residents to the meeting package (scientific sessions, coffee breaks, convention bag and CME certificate) and to benefit from the discounted group rate and the right to purchase the following: accommodation package, tour package, welcome reception and gala dinner.

**The registration fee entitles spouses, children and guests to benefit from the discounted group rate and the right to purchase the following: accommodation package, tour package, welcome reception and gala dinner.

***The registration fee entitles local physicians to the meeting package (scientific sessions, coffee breaks, convention bag and CME certificate).

CANCELLATION POLICY

Request for cancellation must be in writing and mailed, faxed or e-mailed to NAAMA. Refunds, if applicable, will be issued after the end of the convention as follows:

75% refund if cancelled before May 1st, 2012
50% refund if cancelled between May 1st and May 31st, 2012
NO refund if cancelled after June 1st, 2012

Please note that this cancellation policy will be applied to all cancellations with no exceptions. NAAMA has no control over flight cancellations, rescheduling or delays.
